Who Qualifies for a Chronic Special Needs Plan?


To be eligible for a CSNP, individuals must meet certain criteria:

- Have a qualifying chronic condition specified by the plan such as: diabetes, cardiovascular disease, and/or chronic heart failure.

- Be enrolled in Medicare Parts A and B.

- Reside within the plan's service area.




Enrolling in a Chronic Special Needs Plan:

Enrollment in a CSNP follows the same guidelines as enrolling in any Medicare Advantage plan. Beneficiaries can typically join during the annual enrollment period (October 15th to December 7th) or during a special enrollment period if they meet certain qualifying criteria.
